In some cases, wisdom teeth do not erupt. These are called retained teeth. They can also be dystopian, which means they grow in the wrong position relative to the tooth row.

In modern dentistry, there is no consensus on what to do with wisdom teeth. Some doctors insist on their mandatory removal (extraction). Others argue that the "eights" can be useful for the installation of a bridge or braces. But specialists agree with each other that there are situations when they must be removed.

When you need an extraction of a wisdom tooth ("eight")

Indications for removal can be divided into two groups - planned and emergency, or urgent.

Planned include:

  • an atypical position that interferes with the growth of neighboring teeth, making it difficult to eat, leading to damage to the oral mucosa;
  • the presence of a tumor, a root cyst;
  • Difficult teething, which causes the hood over the tooth to become inflamed;
  • Difficulties with treatment of caries, pulpitis, periodontitis of neighboring teeth.

Emergency indications are:

  • severe toothache;
  • a purulent process, which may result in periostitis or osteomyelitis;
  • mechanical trauma, fractured tooth.

How to prepare for surgical wisdom tooth extraction

The surgery is performed after diagnosis, thanks to which the doctor will get information about the exact location of the roots of the "eight". Usually, an aiming photograph and an orthopantogram are taken. In addition, computer tomography is used, which provides higher accuracy.

Before wisdom tooth extraction, it is necessary to stop smoking and drinking alcohol. The fact is that tobacco and alcohol change blood viscosity and other important blood parameters. This can cause swelling and bleeding.

You should also tell your doctor if you are taking medications or supplements. It may be necessary to cancel them for a while. This applies primarily to antibiotics, anticoagulants, anti-inflammatory drugs.

It is advisable to have your teeth professionally cleaned before a scheduled extraction.

Stages of wisdom tooth extraction

The procedure is performed in several stages. Most often, 2-10 minutes is enough to remove an "eight". But the doctor still makes a surgical plan in advance. He also follows protocols to prevent possible complications.

The course of removal depends on whether it is simple or complex. But in any case, the patient is given anesthesia before the operation. Local anesthesia is usually used.

After the extraction, regardless of how it was performed, the doctor washes the hole with antiseptic. If necessary, stitches are placed on it.

Simple extraction - an erupted "eight" with the roots properly positioned

If the third molar has erupted at the correct angle and the configuration of its roots is normal, the extraction is called simple. The doctor uses an elevator and forceps to grasp the "eight", loosen it and pull it out. Thanks to anesthesia, the patient does not feel pain.

Complex - retained and dystopian "eight" teeth

A complex extraction is performed if the third molar:

The following factors influence the complexity of the operation:

  • root shape features;
  • the extent to which the tooth has erupted: fully erupted, retained or semi-retained, i.e. partially hidden by the gum;
  • if there's a tilt and which way;
  • the doctor's skill level.

Features of removal on the lower and upper jaw

Depending on where the wisdom tooth is located, its extraction may have certain specifics. The doctor needs to take this into account during the extraction.

The bone of the upper jaw is softer and more porous. This simplifies the removal of the "eight", helps to perform it faster. But the surgeon should bear in mind that the maxillary sinuses are located nearby, the roots can sprout into them. Therefore, it is necessary to be especially careful not to damage the mucous membrane, which serves as the bottom of these cavities.

The bone of the lower jaw is more dense, so it is more difficult to extract the eights from it. Also, their roots are often irregularly shaped and because of this they braid the canal of the mandibular nerve, complicating the work of the dentist. Therefore, a panoramic image is necessarily performed, and in more complex cases, computerized tomography is used. These methods help the doctor to accurately determine the position of the roots and perform the surgery without any problems.

Care and recovery after removal of the "eight"

Swelling is a normal consequence of third molar extraction. Swelling can be relieved with a cold compress. Warm compresses should not be used, as they will increase swelling.

Also, after removal, you can't:

  • to touch the blood clot that formed in the hole with your tongue;
  • to chew on the side where the extracted tooth was located;
  • Rinse your mouth vigorously - you can swish an antiseptic solution into your mouth, then just hold it in for 20-30 seconds and spit it out.

Necessary:

  • exclude hard, tough foods from the diet for 4-5 days, eat only soft, warm food;
  • no smoking, no alcohol for at least a week;
  • Limit physical activity for 5-7 days after surgery to remove the "eight";
  • avoid overheating, do not visit baths and saunas for 5-7 days.

If the doctor's recommendations are followed, the postoperative hole heals within 1-5 days.

You can brush your teeth on the second day after extraction. It is allowed to use a hard-bristled brush, but be careful not to touch the hole left after extraction.

What complications can there be after removal

Swelling of the soft tissues of the gum and cheek is a normal reaction of the body to the manipulation of the surgeon. It may be accompanied by enlarged lymph nodes and pain when swallowing. As a rule, the swelling goes away on the fourth day after extraction. If it persists longer, inform your doctor.
